Job Description

Position Overview We are seeking a Registered Nurse Care Coordinator to support patients transitioning from hospital care to continued treatment. This remote role focuses on connecting with recently discharged patients, providing clinical guidance, and helping coordinate follow-up care and appointments. Nurses will work closely with patients over the phone to understand their needs, answer questions, and ensure they are directed to the appropriate care resources. Responsibilities Manage inbound calls from patients recently discharged from the hospital seeking continued care and support Handle approximately 4 calls per hour , with conversations typically lasting 5-10 minutes Provide patient education, guidance, and care coordination support through phone-based consultations Assess patient needs and help direct them to the appropriate providers or departments for scheduling and follow-up care Utilize established scripts and workflows while applying clinical judgment to support patient outcomes Collaborate with internal teams to ensure a smooth transition from hospital discharge to ongoing care Required Qualifications Active Bachelor of Science in Nursing (BSN) degree Minimum 2 years of clinical nursing experience working with adult patients (pediatric-only experience does not translate well for this role) Strong verbal and written communication skills with the ability to effectively communicate with patients and healthcare teams Active, unrestricted compact state RN license with primary state of licensure in one of the following: Alabama, Arizona, Arkansas, Colorado, Delaware, Florida, Georgia, Guam, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Mississippi, Missouri, Montana, Nebraska, New Hampshire, New Jersey, New Mexico, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, or Wyoming Preferred Qualifications Prior experience in care coordination, patient navigation, telehealth, triage, or healthcare call center environments Experience supporting patients during care transitions or after hospital discharge Work Environment Fully remote position Company-provided equipment included Must have a reliable home workspace and strong communication skills for virtual patient interactions
