Responsibilities:

Complete annual health risk assessments for Medicare Advantage plan participants Review and evaluate each individuals prior and current medical history, health conditions, and medications Educate patients on their conditions and medications, offering guidance to support self-management Identify gaps in care and recommend steps for follow-up or resolution Focus strictly on assessmentsno need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Missouri BLS (CPR) certification Board Certification (AANP or ANCC) MSN degree from an accredited nursing program Minimum of 6 months experience as a Nurse Practitioner preferred Enjoy competitive compensation of $80$90 per completed evaluation, plus a mileage stipend and administrative time.
