Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes Review both current and historical medical conditions along with medication history Educate patients and families about medical issues and medications Identify care gaps and recommend solutions to promote optimal patient outcomes No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Missouri Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN
