Key Responsibilities:

Perform annual risk assessments for members enrolled in a Medicare Advantage health plan Evaluate prior and current medical history, medical conditions, and review medication usage Provide education to patients on documented health conditions and prescribed therapies Identify any care gaps and offer evidence-based recommendations for follow-up Assessments may be annual check-ups or post-discharge visits; treatment planning or prescribing is not required

Qualifications:
