Key Responsibilities:

Perform in-home annual health risk assessments for Medicare Advantage plan members Review and evaluate patients medical history, ongoing conditions, and medication use Educate patients on chronic conditions, medication management, and preventive health measures Detect gaps in care, providing on-the-spot recommendations to address them No responsibility for developing treatment plans or prescribing medication

Qualifications:

Active Nurse Practitioner license in Missouri Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing school At least 6 months of recent Nurse Practitioner experience preferred In this role, you'll receive $80$90 per completed evaluation, along with stipends for mileage and administrative time.
