Key Responsibilities:

Conduct comprehensive annual risk assessments in the home setting for Medicare Advantage members Review and evaluate prior and current medical history, existing medical conditions, and medication records Deliver patient education on chronic diseases, overall health conditions, and prescribed medications to promote improved health outcomes Identify gaps in care and suggest actionable recommendations Visits may include annual assessments and post-discharge follow-ups No ongoing treatment planning or prescribing requiredfocus is exclusively on assessment and education

Qualifications:

Current and valid Nurse Practitioner license in Missouri Board Certification through AANP or

ANCC CPR

certification (BLS) required Master of Science in Nursing from accredited program At least 6 months of recent Nurse Practitioner experience preferred This opportunity offers $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
