Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in Doniphan, Naylor, Fairdealing, and surrounding areas Evaluate medical history, conditions, and medications to offer a complete clinical picture Deliver education to patients on their diagnoses and medication regimens Identify opportunities to improve patient care and recommend actionable steps to close care gaps No requirements for creating treatment plans or prescribing medications

Qualifications:
