Job Description

Advance your nursing career with a field-based role supporting patients in their homes. This full-time, 13-week assignment is centered in Mississippi State and offers a rewarding blend of autonomy and collaboration, focusing on the delivery of skilled home health care.
Schedule:
Monday Friday, 8:00 AM 5:00
PM Role Highlights:
Field-based care, requiring daily travel to patient homes within an assigned territory Caseload of approximately 30 points per week (typically 35 patient visits daily) Utilization of Home Care Home Base (HCHB) for all electronic documentation Comprehensive patient assessments using
OASIS Essential Qualifications:
Active RN license for the state of Mississippi in good standing Minimum of 1 year of recent experience in Home Health (field work preferred) Demonstrated proficiency with Home Care Home Base (HCHB) Skilled with OASIS assessments and documentation Reliable transportation, valid drivers license, and auto insurance
Key Responsibilities:
Complete thorough OASIS assessments and develop tailored care plans Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and managing chronic conditions Consistently monitor patient progression, noting and addressing changes in condition Educate patients and families on disease processes, medication management, and preventive care strategies Document care promptly in HCHB, ensuring accuracy and regulatory compliance Collaborate with interdisciplinary team members for effective, coordinated patient care Engage in case conferences, staff meetings, and participate in ongoing education
