This is an excellent opportunity for a registered nurse with home health experience to contribute meaningfully to patient outcomes in a field-based setting. This full-time, 13-week contract role focuses on providing skilled care and support to patients in their homes, ensuring continuity and quality of care.

Schedule/Hours:

Monday Friday, 8:00 AM 5:00

PM Role Highlights:

Serve patients within an assigned territory, conducting daily travel to patient homes (mileage reimbursement provided). Expected to manage a caseload of approximately 30 points per week, typically equating to 35 patient visits per day. Utilize Home Care Home Base (HCHB) for all electronic documentation and employ OASIS for comprehensive patient assessments. Develop and implement individualized care plans, performing skilled nursing tasks such as wound care, medication administration, IV therapy, and chronic disease management. Monitor, evaluate, and document patient progress; promptly communicate significant changes to physicians and the care team. Educate patients and families about conditions, medications, and best practices for continued health. Collaborate with interdisciplinary team membersincluding therapists, social workers, and home health aidesensuring coordinated care. Participate in regular case conferences, staff meetings, and ongoing educational opportunities. Maintain compliance with regulatory requirements and documentation standards.

Qualifications:

Active and unencumbered RN license in the state of practice. Minimum of one year of recent home health experience; field experience strongly preferred. Proficiency with Home Care Home Base (HCHB) and OASIS documentation is required. Must have reliable transportation, a valid drivers license, and current auto insurance.
