Schedule:

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members during visits that average around 45 minutes each Review and evaluate patients current and previous medical history, medical conditions, and medication use Offer thorough education to patients about their diagnoses and medications Identify any care gaps and share actionable recommendations to improve patient outcomes Visits may range from annual health assessments to post-discharge reviews

You will not be required to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in North Carolina Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months recent experience as a Nurse Practitioner is preferred Master of Science in Nursing (MSN) from an accredited program
