Key Responsibilities:

Conduct comprehensive home health risk assessments, evaluating medical and medication histories for older adults Meet members in their homes for approximately 45-minute visits, which may be annual health checks or post-discharge assessments Educate patients on managing their medical conditions and proper medication usage Identify care gaps and recommend resources or next steps for resolution Focus exclusively on assessment and patient educationno ongoing treatment planning or prescribing needed

Qualifications:

Active Nurse Practitioner License in North Carolina Board Certification (AANP or ANCC) Masters degree (MSN) from an accredited nursing program Current BLS/CPR certification At least 6 months of recent Nurse Practitioner experience (preferred)
