Key Responsibilities:

Complete annual health assessments and post-discharge evaluations in clients homes Review patients medical and medication histories to assess current and prior medical conditions Educate patients about their health conditions and prescribed medications Identify gaps in care while providing actionable recommendations Focus on thorough, compassionate evaluationsno treatment planning or prescription required

Qualifications:

Active Nurse Practitioner license in North Carolina Board Certification via AANP or ANCC Masters degree in Nursing (MSN) from an accredited institution Current BLS/CPR certification Minimum 6 months experience as a practicing NP preferred
