A highly skilled and compassionate Home Health Registered Nurse (RN) is needed to deliver exceptional patient care in a field-based role. This contract position focuses on providing critical nursing expertise to patients in their homes, working closely with interdisciplinary teams to promote optimal health outcomes.

Schedule:

Monday Friday, 8:00 AM 5:00

PM Key Qualifications:

Active, unencumbered RN license in the state of practice At least 1 year of recent home health nursing experience (field experience preferred) Proficiency in Home Care Home Base (HCHB) electronic documentation OASIS assessment experience required Strong clinical assessment, communication, and organizational skills Reliable transportation, valid drivers license, and proof of auto insurance Ability to work independently and manage a flexible, field-based caseload BSN degree preferred Experience managing a caseload of 2532 points per week and familiarity with Medicare/home health compliance is a plus

Primary Duties:

Conduct thorough OASIS assessments and develop individualized care plans for patients Deliver skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease monitoring Monitor patient progress, update care plans, and communicate status changes effectively to the care team Provide education and support to patients and their families on treatments and preventative care Ensure accurate and timely clinical documentation using HCHB Collaborate with therapists, medical social workers, and aides to ensure continuity of care Participate in interdisciplinary case conferences and ongoing professional development Uphold all home health and agency regulatory standards
