Key Responsibilities:

Complete thorough annual home health risk assessments for Medicare Advantage members in their homes Review and evaluate medical history, current conditions, and medication use Educate patients on disease management, medication adherence, and healthy living Identify care gaps and recommend solutions to optimize health outcomes Required documentation onlyno treatment planning or medication prescribing

Qualifications:

Active Nurse Practitioner license in North Carolina Current CPR certification (BLS) Board Certification (AANP or ANCC) Graduation from an accredited MSN program Minimum of 6 months of recent NP experience preferred
