Schedule/Hours:

Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage health plan members in residential settings Evaluate prior and current medical history, medical conditions, and medication history Educate patients regarding their medical conditions and medication regimens Identify gaps in care and provide actionable recommendations for resolution No treatment plan creation or medication prescribing required Assessments typically take approximately 45 minutes each

Qualifications:

Active Nurse Practitioner license in North Carolina Current BLS/CPR certification Board certification through AANP or

ANCC MSN

from an accredited nursing program At least 6 months of current Nurse Practitioner experience preferred
