Complete comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients past and current medical histories, conditions, and medications Deliver clear education to patients on their health conditions and prescribed therapies Detect gaps in care, offering recommendations to improve patient outcomes Focus exclusively on assessmentsno treatment planning or prescribing involved

Qualifications:

Active Nurse Practitioner License in North Carolina MSN degree from an accredited nursing school Board Certification via AANP or ANCC Current CPR certification (BLS) At least 6 months of recent Nurse Practitioner experience preferred This role compensates $80-$90 for each completed evaluation, and also provides a mileage stipend plus pay for administrative time.
