Key Responsibilities:

Conduct in-depth annual home health risk assessments for Medicare Advantage members Evaluate patients prior and current medical history, conditions, and medications Educate members on their health conditions and medication regimens Identify gaps in care and provide clear recommendations to optimize health outcomes No responsibility for treatment planning or prescribing medications

Desired Qualifications:

Active Nurse Practitioner License in North Carolina Board Certification through AANP or

ANCC MSN
