This opportunity is ideal for experienced Nurse Practitioners passionate about personalized patient care outside a traditional clinical setting. Work within Taos County, including areas such as Taos, Questa, and Red River, providing valuable healthcare assessments for Medicare Advantage plan members.

Key Responsibilities:
Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members in their homes Review and evaluate medical history, current conditions, and medication use Educate patients on managing their health conditions and proper medication usage Identify care gaps and deliver recommendations to help improve patient outcomes Focus solely on assessmentno requirement for treatment planning or prescribing medications
Desired Qualifications:
Active Nurse Practitioner license within NM Master of Science in Nursing from an accredited institution Board Certification through AANP or ANCC Current CPR certification (BLS) At least 6 months of recent Nurse Practitioner experience preferred Enjoy competitive compensation of $80-$90 per completed evaluation, with additional mileage stipend and compensation for administrative time.
