Job Description

Seeking a skilled Nurse Practitioner to perform home health risk assessments on Medicare Advantage plan members throughout Grant County. This per diem opportunity provides outstanding flexibility and the chance to make a meaningful impact in patient care, with visits to areas such as Bayard-Santa Rita, Santa Clara, Tyrone, Hurley, and Roswell.
Schedule:
Flexible shifts available, Monday through Sunday Self-selected schedulingchoose the days and times you work Minimum hourly commitment on a weekly or bi-weekly basis
Responsibilities:
Complete home health risk assessments, including annual wellness and post-discharge visits Review and evaluate patients medical and medication histories Educate patients on diagnosed conditions and prescribed medications Identify gaps in care and make recommendations to address them Documentation and reporting as requiredno treatment planning or prescription writing necessary
Qualifications:
Active Nurse Practitioner license in New Mexico Board Certification through AANP or ANCC Current BLS (CPR) certification MSN from an accredited nursing school At least 6 months of current experience as a Nurse Practitioner (preferred) Enjoy highly competitive, evaluation-based compensation$80-$90 for each risk assessment completed, plus mileage reimbursement and compensation for administrative time. This position is ideal for an experienced NP seeking autonomy, flexible scheduling, and a focus on patient education and care review, without treatment plan management or prescribing.
