You'll play a crucial role in care continuity by performing in-depth annual or post-discharge health assessments during home visits. Schedule Flexible shifts available Monday through Sunday Self-scheduling to match your availability Minimum weekly or bi-weekly hour commitment Key Responsibilities Complete comprehensive home health risk assessments for Medicare Advantage plan members Review and evaluate prior and current medical history, including all medical conditions and medication regimens Educate patients regarding their diagnoses and medication management Identify care gaps and recommend steps for closure No development of treatment plans or medication prescribing responsibilities Qualifications Active Nurse Practitioner license in New Mexico Current CPR certification (BLS) Board Certification from AANP or ANCC Completion of MSN from an accredited nursing school Minimum 6 months of nurse practitioner experience preferred Compensation Enjoy competitive earnings of $80$90 for each evaluation you complete, plus additional mileage reimbursement and compensation for administrative time.
