Job Description

Step into a meaningful role providing advanced care for Medicare Advantage members through home health risk assessments within Taos County. As a Nurse Practitioner specializing in home health, you will deliver comprehensive assessments that help improve outcomes for vulnerable populations across communities such as Taos, Questa, and Red River.
Schedule:
Flexible shifts available seven days a week Self-scheduling allows you to build a calendar that fits your lifestyle Minimum weekly or bi-weekly commitment required
Key Responsibilities:
Conduct thorough annual and post-discharge risk assessments in members homes, averaging 45 minutes per visit Evaluate medical histories, current health status, and medication regimens to paint a complete clinical picture Educate patients about their medical conditions, treatment options, and medication usage Identify gaps in care, providing actionable recommendations to close those gaps Focus exclusively on assessmentno requirement for prescribing medications or developing treatment plans
Qualifications:
Active and unencumbered Nurse Practitioner License in New Mexico Board certification from AANP or ANCC Current Basic Life Support (BLS/CPR) certification MSN degree from an accredited nursing program Preferably 6 months or more of recent Nurse Practitioner experience, ideally in home health or risk evaluation In this per diem position, you will be compensated $80-$90 per completed evaluation, with mileage and administrative time also supported.
