Job Description

Seeking a skilled Nurse Practitioner to perform home health risk assessments throughout Rio Arriba County, NM. This per diem opportunity provides the flexibility to create your own schedule, with shifts available any day of the week to best accommodate your needs. Locations served include Espanola, Chama, and Abiqui, with the ability to be scheduled at any of these areas during your shift.
Schedule:
Flexible shifts, Monday through Sunday Create your own schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform in-home health risk assessments for Medicare Advantage health plan members Evaluate prior and current medical history, medical conditions, and medication usage Deliver patient education regarding health conditions and prescribed medications Identify any gaps in care and recommend appropriate next steps No requirement for treatment planning or prescribing medication
Qualifications:
Active Nurse Practitioner license in NM Current CPR certification (BLS) Board Certification (AANP or ANCC) 6 months or more of current NP experience preferred Master of Science in Nursing (MSN) from an accredited program This role offers competitive compensation of $80-$90 for each completed evaluation, with additional stipends for mileage and administrative time.
