Job Description

Deliver critical care to patients in the comfort of their own homes by providing comprehensive risk assessments as a Nurse Practitioner on a per diem basis. This role centers on conducting home health risk evaluations for Medicare Advantage plan members throughout San Miguel County, including Las Vegas, Pecos, and Montezuma. Assessments typically last 45 minutes and may range from annual health evaluations to post-discharge visits, depending on patient needs.
Schedule:
Flexible shifts available any day of the week (MondaySunday) Build your own schedule Minimal weekly or bi-weekly commitment required
Key Responsibilities:
Conduct annual home health risk assessments for a Medicare Advantage health plan Review and evaluate prior/current medical history, conditions, and medications Educate patients on managing medical conditions and understanding medications Identify gaps in care and advise on ways to address these issues No need to create treatment plans or prescribe medications; focus on assessment and recommendations
Required Qualifications:
Active Nurse Practitioner license in NM Current CPR certification (BLS) Board certification via AANP or
ANCC MSN
from an accredited nursing program At least 6 months of current Nurse Practitioner experience is preferred Utilize your advanced clinical skills to provide vital assessments and empower patients in their health journeys. Compensation ranges from $80 to $90 per evaluation, with additional stipends for mileage and administrative efforts, supporting your dedication as you travel within San Miguel County.
