Key Responsibilities:

Conduct comprehensive in-home and telehealth patient assessments Complete UAS-NY assessments accurately and efficiently Evaluate patients' medical, functional, behavioral, and care needs Assess eligibility and needs for appropriate long-term care services Develop and document detailed clinical assessments Collaborate with interdisciplinary healthcare teams and community providers Educate patients and families regarding available care and support services Maintain accurate and timely clinical documentation Advocate for patient-centered care and appropriate healthcare resources Requirements Active New York RN License Minimum 2 years of clinical experience Experience in Home Health.
