Job Description

Join Our Team as a Part-Time Nurse Practitioner Program. Are you passionate about supporting youth, and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 120 impactful programs across Long Island and NYC, is looking for a dedicated part time Nurse Practitioner to join the Home Base Community/ Intensive program.

7 Hours (flexible hybrid work schedule)

320 Carleton Ave in Central Islip, NY What drives the core purpose of this job: The Nurse Practitioner will be responsible for conducting comprehensive mental health assessments to evaluate the emotional, psychological, and behavioral well-being of youth in the Home Base program. This includes prescribing and managing psychiatric medications appropriate to support individualized treatment plans.
What are the key responsibilities:
Conduct thorough mental health assessments for youth to diagnose and evaluate emotional, psychological, and behavioral conditions. Monitor patients' progress, adjusting treatment plans as necessary to ensure optimal care. Collaborate with the Home Base multidisciplinary team, including Family Communication Specialists, Case Managers, and other healthcare professionals, to provide comprehensive care. Educate youth and families about mental health conditions, treatment options, and medication management. Maintain accurate and up-to-date patient records in compliance with healthcare regulations and confidentiality standards. Develop and implement treatment plans, including prescribing and managing psychiatric medications as appropriate. Maintain progress notes and accurate case records into the computer system with all necessary data entry in accordance with contract mandates. Participate in team meetings, case conferences, agency presentations and trainings as required. Serve as a resource to case managers in specialty areas of mental health. Administration. What key knowledge, skills, and abilities are required to excel in this role: 1. Master's or Doctoral degree in Nursing from an accredited program, with specialization in Psychiatric-Mental Health Nursing. 2. Current New York State RN license and New York State certification and registration as a Nurse Practitioner in Psychiatry. 3. More than 3,600 hours of qualifying Nurse Practitioner practice experience required, allowing independent NP practice in New York State without a written collaborative practice agreement with a physician. 4. Experience working with at-risk youth and families, including youth involved with juvenile justice, child welfare, schools, or other community-based systems, preferred.
