Complete comprehensive annual home health risk assessments for Medicare Advantage plan members in locations throughout Albany County Review patient medical histories and current health conditions during approximately 45-minute visits Provide patient education regarding chronic conditions and medications Assess for potential gaps in care and make recommendations, supporting continuity of care Visits may varyfrom annual health assessments to post-discharge follow-ups No responsibility for treatment planning or medication prescribing

Qualifications:

Active Nurse Practitioner license in New York State Masters degree in Nursing (MSN) from an accredited institution Current Board Certification through the AANP or ANCC Current CPR certification (BLS) At least 6 months of recent nurse practitioner clinical experience preferred This per diem position offers highly competitive compensation of $80-$90 per completed evaluation, with a mileage stipend and additional pay for administrative time.
