Job Description
Job Summary We are seeking a dedicated and compassionate Field Nurse (Registered Nurse - RN) to assess or reassess the patients in their own environments. This energetic role involves delivering personalized nursing care, conducting comprehensive assessments, and supporting patients across diverse settings. Services needed in the five boroughs of New York City. Duties Deliver comprehensive hospice nursing care, including assessments, symptom management, and care planning Coordinate with interdisciplinary team members (social workers, chaplains, aides, physicians) Educate patients and families on disease progression, comfort measures, and available resources Maintain accurate, timely clinical documentation Participate in case conferences and care‑planning meetings Provide on‑call support if required (specify if applicable) Requirements Active RN license in the state of practice Minimum 1 year of hospice nursing experience (more preferred) Strong clinical assessment and critical‑thinking skills Ability to work independently in the field Reliable transportation and valid driver's license Join us as a Field Nurse (RN) Home Care to deliver compassionate clinical excellence directly into patients' homes!
