The ideal candidates will have home care experience and be responsible for: Conducting admission assessments and biannual reassessments for patients. Establishing and providing a plan of care for the patient based on the results of the nursing assessment visit, client needs, and physician orders. Providing education to patients/families and paraprofessional staff. Conducting as-needed visits to patients who require additional support. Communicating changes in patients' conditions, treatment, and medications directly to the agency.

Nurse Requirements:

Graduate of an accredited school of nursing (ADN or BSN). Home care experience preferred. Must have a current NYS license and registration to practice as a Registered Professional Nurse in New York State. Must have a reliable vehicle to travel to patients. This position requires travel throughout Sullivan County and neighboring counties. (mileage reimbursement provided). Bilingual (Spanish) preferred but not required. Candidates who are bilingual may be eligible for a higher rate of pay.
