DETAILS Flexible Scheduling:

Hourly and per diem opportunities available Initial visits available Flexible scheduling based on your availability Opportunities available throughout Suffolk County Home-care assignments with individualized client care

RESPONSIBILITIES

Provide skilled nursing care in the client's home Complete nursing assessments and monitor changes in client condition Develop and follow individualized care plans Coordinate care with clients, families, caregivers, and other members of the healthcare team Provide education and support to clients and their families Document patient care accurately and timely Recognize changes in condition and communicate concerns appropriately Provide compassionate, patient-centered care

QUALIFICATIONS

Valid New York State Registered Nurse (RN) license required Home care experience preferred Strong clinical assessment and communication skills Comfortable working independently in a home-care environment Reliable, professional, and patient-centered Must be authorized to work in the U.S. Must have updated medicals/vaccinations or be willing to obtain them Valid driver's license and reliable transportation preferred Completed drug screen or completed Habituation Form Must successfully complete the Alvita Care onboarding process before accepting cases
