Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review medical history, existing conditions, and medication regimens Educate patients about their conditions and prescribed medications Identify and address gaps in care, offering recommendations for follow-up Focus on assessmentno need to create detailed treatment plans or prescribe medications

Qualifications:
