Key Responsibilities:

Conduct comprehensive home health risk assessments (approximately 45 minutes per visit) Evaluate medical histories, existing conditions, and medication usage Educate patients about their health conditions and medications Identify care gaps and provide actionable recommendations for improvement No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Ohio Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN
