Essential Responsibilities:

Complete annual health risk assessments at patient homes in areas such as Ashtabula, Conneaut, and Geneva Review prior and current medical history, including medication use and pre-existing conditions Offer patient education regarding medical conditions and medication management Detect possible gaps in patient care and suggest appropriate recommendations No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner (NP) license in Ohio Board Certification through AANP or ANCC Current CPR (BLS) certification MSN from an accredited nursing school At least 6 months of current NP experience preferred In return for your expertise and compassionate care, youll receive a competitive rate of $80-$90 per completed evaluation plus a mileage stipend and additional compensation for administrative time.
