Schedule and Setting:

Monday Friday, 8:00 AM 5:00 PM Field-based position: daily travel to patient homes within your assigned territory Caseload expectation: 30 points per week (typical range is 35 patient visits per day) Electronic charting through Home Care Home Base (HCHB) OASIS assessment tool used for patient evaluations

Qualifications:

Valid RN license in good standing for the state of practice Minimum 1 year of recent home health nursing experience; field-based care highly preferred Proficient with Home Care Home Base (HCHB) documentation Skilled in conducting and documenting OASIS assessments Reliable vehicle, valid drivers license, and current auto insurance are required

Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management Monitor patient condition, recognize changes, and swiftly communicate with interdisciplinary teams and physicians Educate patients and families on diagnoses, medications, and self-care strategies Ensure thorough, timely documentation in HCHB Work alongside therapy teams, social work, and home health aides for integrated, coordinated care Participate in case conferences, staff meetings, and pursue ongoing professional education Comply with all standards and regulations governing home health care
