A full-time, 13-week contract opportunity is available for an experienced Registered Nurse specializing in home health care. This field-based position is ideal for professionals skilled in providing hands-on patient care and comfortable with electronic documentation systems. The work schedule is Monday through Friday, 8:00 AM to 5:00 PM, with expectations of 35 patient visits daily (averaging a caseload equivalent to 30 points per week).

Key Responsibilities:

Conduct comprehensive OASIS assessments and design individualized care plans. Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management in patient homes. Monitor and evaluate patient conditions, identify status changes, and communicate relevant updates to the care team and physicians. Educate patients and families on disease management, treatments, medications, and preventive care strategies. Ensure accurate, timely documentation using Home Care Home Base (HCHB). Collaborate closely with therapists, social workers, and aides for integrated care delivery. Participate in staff meetings, case conferences, and required ongoing education. Maintain compliance with regulatory standards and best practices for home health nursing.

PM Qualifications:

Active RN license (state of practice), in good standing. At least one year of recent home health nursing experience, with strong fieldwork background preferred. Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS assessments. Reliable personal transportation with valid drivers license and current auto insurance.
