Ensure timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and care plans. Collaborate with the interdisciplinary team to develop and update individualized resident care plans. Monitor Medicare and Medicaid documentation to support appropriate reimbursement. Review clinical documentation for accuracy and completeness. Ensure compliance with federal, state, and facility regulations. Participate in quality assurance and performance improvement initiatives. Prepare for state surveys and assist with regulatory compliance activities. Educate and support nursing staff regarding MDS documentation requirements and best practices. Maintain accurate records and meet all assessment submission deadlines. Qualifications Current (RN) or (LPN) license in good standing. Previous MDS Coordinator experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting preferred. Strong knowledge of

RAI, MDS

3.0, Medicare, Medicaid, and PPS guidelines. Experience with care planning, reimbursement, and regulatory compliance. Excellent assessment, organizational, and communication skills. Ability to work collaboratively with an interdisciplinary team. Proficiency with electronic medical record (EMR) systems.
