Key Responsibilities:

Conduct detailed annual home health risk assessments for Medicare Advantage members Assess prior and current medical histories, review medical conditions, and evaluate medication regimens Educate patients regarding their health conditions and prescribed medications Identify care gaps and suggest recommendations for improved patient outcomes Focus exclusively on assessments; no treatment plan development or medication prescribing required

Qualifications:

Current Nurse Practitioner license in Pennsylvania Board Certification through AANP or ANCC Current BLS (CPR) certification MSN degree from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner preferred
