Schedule & Setting:

Monday through Friday, 8:00 AM 5:00 PM Caseload of approximately 30 points per week (typically 35 patient visits per day) Daily travel to patient residences within assigned territory (mileage reimbursement provided) Electronic documentation using Home Care Home Base (HCHB) Utilize OASIS as the primary assessment tool

Key Responsibilities:

Conduct thorough OASIS assessments and develop personalized care plans Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, recognize changes in condition, and communicate updates to physicians and interdisciplinary team members Educate patients and caregivers on treatment, medications, home safety, and preventive care Maintain comprehensive and timely documentation in HCHB Collaborate with a multidisciplinary team of therapists, social workers, and aides to ensure cohesive patient care Participate in case conferences, staff meetings, and ongoing education sessions Ensure full compliance with regulatory and agency standards

Desired Qualifications:

Active RN license in good standing in the state of practice Minimum 1 year of recent Home Health field experience required Demonstrated expertise in Home Care Home Base (HCHB) and OASIS documentation Reliable vehicle, current drivers license, and up-to-date auto insurance
