Job Overview We are seeking a dynamic and detail-oriented Registered Nurse Assessment Coordinator to join our dedicated healthcare team. In this vital role, you will coordinate comprehensive patient assessments, ensure compliance with healthcare regulations, and facilitate seamless communication among staff and residents. Your expertise will support optimal patient care outcomes while maintaining adherence to industry standards and documentation protocols. is seeking an organized and knowledgeable Registered Nurse Assessment Coordinator (RNAC) to lead MDS completion, case mix management, care planning, and staff education in our skilled nursing facility. This role works closely with nursing leadership, therapy, and the interdisciplinary team to ensure assessments accurately reflect each resident's needs and the care provided. Key Responsibilities Coordinate accurate and timely completion of MDS assessments and required submissions. Manage assessment schedules and monitor completion deadlines. Review clinical documentation to support accurate coding, case mix, and reimbursement. Collaborate with nursing, therapy, dietary, social services, and other departments to capture resident conditions, services, and changes in status. Coordinate individualized, person-centered care plans and interdisciplinary care conferences. Ensure care plans reflect assessment findings, resident preferences, and current care needs. Educate staff on MDS requirements, documentation practices, care planning, and changes affecting assessment and reimbursement. Conduct documentation audits, identify opportunities for improvement, and provide follow-up education. Participate in quality improvement activities and survey preparation related to assessments and care planning. Communicate assessment, documentation, and case mix concerns to the Administrator and Director of Nursing. Qualifications Current, active Pennsylvania Registered Nurse license. Experience in skilled nursing or long-term care. Working knowledge of MDS assessments, the RAI process, care planning, PDPM, and Medicaid case mix. Strong assessment, organization, communication, and staff education skills. Ability to work independently and collaborate with an interdisciplinary team. Experience with electronic health records; PointClickCare experience preferred. Prior
