2987668741-2 Apply for this job now Job Description Matrix Medical Network is seeking a compassionate Nurse Practitioner to provide in-home health assessments and support quality patient care. In this role, you will conduct comprehensive evaluations, coordinate care plans, and collaborate with multidisciplinary teams to improve outcomes and reduce healthcare costs. You'll leverage your clinical expertise to support health management initiatives for insurance plans and government agencies. Responsibilities Perform comprehensive in-home and community-based health assessments for adult and geriatric patients. Obtain detailed medical histories, conduct physical examinations, and identify acute and chronic health issues. Order and interpret appropriate diagnostic tests within scope of practice and according to clinical guidelines. Develop and implement individualized care plans focused on disease management, prevention, and functional improvement. Provide medication reconciliation, basic medication management, and patient education in collaboration with prescribing providers. Identify and close care gaps by coordinating with primary care physicians, specialists, health plans, and community resources. Document all encounters accurately and completely in the electronic health record (EHR), adhering to regulatory and organizational standards. Educate patients, families, and caregivers on diagnoses, treatment options, self-management strategies, and community support services. Collaborate with interdisciplinary team members to support quality improvement initiatives and optimize patient outcomes. Participate in case reviews, team meetings, and ongoing training to stay current with evidence-based practice and regulatory requirements. Utilize telehealth and remote monitoring tools, when applicable, to follow up with patients and support care continuity. Travel locally to patient homes and community sites, managing schedules to ensure timely and efficient visit completion.
