In this vital role, you will oversee the accurate and timely completion of Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations, including CMS guidelines. Your expertise will directly impact patient care quality, reimbursement accuracy, and regulatory adherence. This position offers an exciting opportunity to utilize your clinical documentation skills and healthcare knowledge to improve resident outcomes and support interdisciplinary collaboration. Duties Coordinate and facilitate comprehensive MDS assessments for long-term care residents, ensuring completion within required timeframes. Review medical records, clinical documentation, and care plans to ensure accuracy and completeness in accordance with clinical documentation standards. Collaborate with nursing staff, physicians, therapy teams, and case managers to gather necessary information for accurate MDS coding and documentation. Ensure compliance with all relevant regulations including Medicare, Medicaid, HIPAA, and state healthcare policies. Utilize the electronic health record (EHR) system to document assessments and update patient records efficiently. Perform documentation review for
ICD-10/ICD
coding, CPT coding, DRG assignment, and utilization management to optimize reimbursement and quality Maintain detailed records of assessments, coding decisions, and regulatory audits while supporting clinical documentation improvement initiatives. Requirements Proven experience with MDS coordination in a long-term care or skilled nursing facility setting. Strong knowledge of CMS regulations, Medicare requirements, and long-term care regulations. Familiarity with Point, Click, Care EMR/EHR system; proficiency in Microsoft Office is preferred. In-depth understanding of medical terminology, anatomy, physiology, ICD-10/ICD coding (ICD-9 knowledge is a plus), CPT coding, DRG assignment, and clinical documentation standards. Excellent organizational skills with the ability to manage multiple assessments simultaneously while maintaining accuracy. Knowledge of HIPAA regulations and state healthcare policies related to medical records management. Effective communication skills to collaborate across multidisciplinary teams including nursing homes, home care agencies, hospice providers, emergency medicine units, or outpatient clinics as needed.
