PRN LTAC RN
providing high-acuity care to medically complex patients in a long-term acute care setting. You'll perform thorough assessments, manage ventilators and complex drips, administer medications, and coordinate care with an interdisciplinary team. Responsibilities Deliver compassionate, evidence-based nursing care to medically complex LTAC patients on a PRN basis Perform assessments, administer medications, manage ventilators, and monitor vital signs Collaborate with physicians, therapists, and interdisciplinary team to develop and update care plans Educate patients and families on conditions, treatments, and discharge instructions Maintain accurate, timely documentation in EMR and follow all facility and regulatory policies Respond promptly to changes in patient condition and participate in emergencies and rapid responses Follow infection control, safety, and quality standards to support excellent patient outcomes Communicate effectively with staff, leadership, and Gifted Healthcare team regarding assignments and needs Required Skills Ventilator management Tracheostomy care IV therapy and infusion management Cardiac and hemodynamic monitoring Wound and skin integrity management Acute and LTAC patient assessment Medication administration and titration Infection control and isolation precautions Electronic Medical Record (EMR) documentation Patient and family education
