Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their residences Assess prior and current medical history, ongoing conditions, and medication usage Educate patients on existing medical conditions and prescribed medications Identify and document gaps in care, providing actionable recommendations to address them No responsibility for treatment planning or medication prescribing

Desired Qualifications:

Active Nurse Practitioner license in Tennessee Board Certification through AANP or ANCC Current BLS CPR certification MSN from an accredited nursing program A minimum of 6 months' experience as a Nurse Practitioner preferred Competitive compensation is provided, with $80$90 per evaluation, including a mileage stipend and pay for administrative time.
