Key Responsibilities:

Perform comprehensive home health risk assessmentseach assessment generally lasts around 45 minutes Assess patients medical history, current conditions, and medication usage Offer education to patients on their conditions and therapies Identify and address gaps in care, providing recommendations as needed Conduct various types of visits, including annual health assessments and post-discharge check-ins No creation of treatment plans or medication prescribing involved

Qualifications:

Active Nurse Practitioner license in Tennessee Board Certification (AANP or ANCC) Current CPR certification (BLS) Minimum of six months' current experience as a Nurse Practitioner preferred MSN degree from an accredited nursing program
