Key Responsibilities:

Conduct comprehensive home-based risk assessments for Medicare Advantage members, typically lasting about 45 minutes Evaluate prior/current medical histories, all medical conditions, and medication regimens Educate members regarding their diagnoses and prescribed treatments Identify gaps in care and offer practical recommendations for health improvement No development of treatment plans or medication prescribing required

Qualifications:
