Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes Review and evaluate prior and current medical history, chronic medical conditions, and medication usage Provide patient education regarding medical conditions and prescribed drugs Identify any gaps in patient care and make recommendations for their resolution Complete thorough documentation; no treatment planning or medication prescribing required

Qualifications:

Active and unrestricted Nurse Practitioner license in Tennessee Current CPR (BLS) certification Board certified through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of current Nurse Practitioner experience (preferred) Compensation for this role includes $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
