Sweetwater Hospital Association is seeking a full time Home Health RN to join our healthcare team. The Home Health RN provides compassionate, quality nursing care to patients in their homes while promoting independence, safety, and overall well-being.

Responsibilities:

Provide skilled nursing care to patients in their homes in accordance with established standards and individualized care plans Assess patient conditions and monitor changes in health status Administer medications and treatments as ordered Educate patients and families on medications, treatments, disease management, and appropriate home care Communicate effectively with physicians, patients, families, and other members of the healthcare team Accurately document patient assessments, care provided, and changes in condition Coordinate patient care and follow established plans of care Complete required clinical and administrative documentation Promote patient safety and independence within the home environment Maintain compliance with hospital policies, procedures, and applicable state and federal regulations

Education:

Graduate from an accredited school of nursing with a current license in the State of Tennessee.

Experience:

Experience is preferred; however, all employees will go through department-specific orientation.
