Job Description

MDS Coordinator Obion County Nursing and Rehab — Union City, Tennessee Position Summary The MDS Coordinator is responsible for coordinating and completing federally required Minimum Data Set (MDS) assessments for residents, ensuring accurate documentation, timely completion, and compliance with Medicare, Medicaid, CMS, and state requirements. Essential Duties Coordinate the MDS assessment process for all assigned residents. Complete and submit MDS assessments accurately and within required timeframes. Review resident medical records, nursing documentation, therapy notes, physician orders, and other clinical information. Conduct resident assessments and interviews as required. Coordinate with nursing, therapy, dietary, social services, activities, and other departments to ensure accurate MDS information. Develop and update resident care plans based on MDS findings. Participate in care-plan meetings and communicate resident needs to the interdisciplinary team. Monitor documentation for accuracy and completeness. Identify changes in residents' conditions and ensure appropriate documentation and follow-up. Maintain compliance with CMS, Medicare/Medicaid, state regulations, and facility policies. Assist with Medicare/Medicaid documentation and reimbursement requirements. Monitor MDS schedules and ensure assessments are completed on time. Educate nursing staff regarding MDS documentation and coding requirements. Maintain confidentiality of resident information. Perform other duties as assigned by the Director of Nursing or Administrator. Qualifications Current Tennessee RN or LPN license preferred/required according to facility requirements. Previous long-term-care or skilled-nursing experience preferred. MDS/RAI experience strongly preferred. Knowledge of Medicare/Medicaid requirements and nursing-home regulations. Strong documentation, organization, and communication skills. Ability to work effectively with an interdisciplinary team.
