Job Overview Hospice Registered Nurse (RN) Case Manager to provide comprehensive end-of-life care coordination for patients in a hospice setting. The ideal candidate will possess extensive clinical expertise, including critical care, ICU, emergency medicine, and post-acute care experience. As a Hospice RN Case Manager, you will serve as a vital link between patients, families, and interdisciplinary teams to ensure quality, patient-centered care that respects individual wishes and promotes comfort during life's final stages. Responsibilities Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs, utilizing strong knowledge of physiology and medical terminology. Develop individualized care plans aligned with patient goals and hospice guidelines, coordinating with physicians, social workers, chaplains, and other team members. Manage symptom control and medication administration with precision, ensuring comfort while adhering to HIPAA regulations and safety protocols. Perform ongoing monitoring of vital signs, disease progression, and response to treatments within inpatient, outpatient, or home settings. Qualifications Valid Texas RN licensure. 2 years Hospice experience

Pay:

$80,000.00

•$90,000.00 per year
