Responsibilities Conduct comprehensive patient assessments to determine eligibility for hospice services, including evaluating physiological status and medical history. Manage intake procedures efficiently by coordinating with healthcare providers, families, and interdisciplinary teams to facilitate timely admissions. Review and interpret medical records, diagnostic evaluations, and electronic health records (EHR) to inform care planning. Collaborate with hospital staff, discharge planners, and case managers to ensure smooth transitions from inpatient or outpatient settings into hospice care. Educate patients and families about hospice services, care plans, and symptom management options with clarity and empathy. Maintain compliance with HIPAA regulations and health policy standards while managing sensitive medical information securely. Document all admission activities accurately in medical records and ensure adherence to healthcare regulations and policies related to patient care. Experience Proven critical care experience within hospital settings such as ICU, emergency room, or medical-surgical units. Experience working in Level I or Level II trauma centers, demonstrating ability to handle high-acuity cases efficiently. Background in post-acute care environments including inpatient, outpatient, or clinic settings with a focus on patient assessment and discharge planning. Knowledge of hospital medicine, infectious disease care, behavioral health, geriatrics, pediatrics, or managed care is highly desirable. Familiarity with electronic health records (EHR) management systems and medical terminology essential for accurate documentation. Strong understanding of health regulation policies including HIPAA compliance and utilization management principles. Ability to manage multiple priorities effectively while providing exceptional patient service in fast-paced environments such as emergency medicine or hospital inpatient units.
