About us Serenity Care Solutions for Hospice is a Fast Growing Hospice in Houston, TX. Primary function is to administer skilled nursing care to hospice patiens and families in their home. Case Manage and Coordinate care with the interdisciplinary team , patient/family and the referring agency. Perform assessment and assistance with care of the terminal illness. Inform physician and other personnel of changes in client needs and family needs. Assess the ability of the caregiver to manage the client care. Responsibilities Collaborate with physicians and other professionals to plan care No- Oncall Make frequent visits to the patient to evaluate their condition Provide support and comfort according to individual patient's needs Work with caregivers to plan and execute excellent patient care Help in administering medication Alleviate sudden symptoms of the disease's end phase Notify a doctor for a patient's condition when deemed necessary Assist patients and their families with paperwork Maintain accurate records Help relatives cope with the upcoming loss and provide post-mortem support
